TUITION ELECTRONIC FUNDS TRANSFER AUTHORIZATION FORM FOR 2010-2011

TUITION AND RELATED FINANCES
The following schedule sets forth the 2010-2011 parishioner's tuition rates, which include books, and other fees set by the School Board in conjunction with this school year's budget. Fees not included are Sacramental fees, F.S.A. dues, graduation fees, and the cost of milk.

Tuition and fees are paid in ten installments, the first two of which are due in August. The remaining installments are due monthly as scheduled in the payment coupon book. Parents are responsible for keeping their account current.  As stated in the school's policies, "the tuition charge for each child in each family will be born by the parent (or guardian) by whom the child is registered and with whom the child
resides". (SFX School Board Policy 5221.3) Further, "All accounts with a balance due as of May 15th will
be treated as delinquent accounts. Transfers and diplomas will not be issued, and registration for the
following school year will not be accepted if a delinquency continues without entering into a payment plan
with the Administration." (SFX School Board Policy 5 .11)

o Refunds
"If a student withdraws prior to the 10th calendar day of the month, fifty (50%) percent of the tuition
charged for that month is refundable.  If a student withdraws on the 10th day of the month or anytime
thereafter, the tuition for the month is fully earned.  

"Registration fees for families currently enrolled at SFX are non-refundable. A non-refundable registration
fee will also apply to families not currently enrolled at SFX, payable at the time of application. The amount
of this fee will be set by the Budget Committee of the School Board and shall be great enough to ensure a
reserved space for a new enrollee. If the enrollee begins attendance at SFX, a percentage of such fee, set by
the Budget Committee, will be allocated to the first month's tuition for the family of the enrollee, but shall
not be subject to the Tuition Refund Policy set forth above." (SFX School Board Policy 1001)
